Are "morning bathers" unsanitary?

I am confused about morning bathers. Do morning bathers usually go to bed without bathing first? I can't imagine working all day, using public toilets, and being outside for a portion of the day to come lay down in your own filth...and even with a spouse. Am I missing something?
